Guidelines for Ordering Controlled Medical Supplies

Business Clients
Businesses may purchase certain items (Syringes, Needles, etc.), for use in their day to day business activities without a prescription by supplying us the Federal Tax ID# for your business.

Consumers
BRICO Medical Supplies, Inc. requires all Consumers wanting to purchase controlled medical supplies, to furnish a prescription that authorizes them to possess and use these items.

*Requirements for Ordering Syringes and Needles; to include Diabetic Supplies

The following States: (CA, CT, DE, FL, IL, ME, NV, NH, NJ, NY, PA, VA), have restrictions that require us to have you fax us copies of your "PRESCRIPTION" for the items you wish to purchase. In addition, in order for us to comply with "Age Verification Requirements", you must also fax us a copy of your "Driver's License".

*We will need to have both of these items faxed to us prior to our shipping your order.

ALL other States do not require a prescription for you to place your order. However, you will still need to fax us your "Drivers License" in order for us to comply with "Age Verification Requirements", prior to us shipping your order.

**Ideally, the Drivers License Address should match the "Ship To" Address of the Order!**

*We reserve the right to choose not to satisfy or fulfill any order, at any time, without needing to offer a reason. If your order is NOT Accepted, you will be phoned or emailed in a timely manner to inform you of our decision.

Prescriptions & Drivers Licenses' should be faxed to 732-321-1542.

(Prescriptions written "PRN" by your physician are valid for 1 year and will be held on file for subsequent orders)

*You are responsible for determining what guidelines you need to follow in your state with regard to syringe and needle possession and usage.

Payment Terms:
We accept Visa/MasterCard/American Express credit cards.

Business Customers that want Net 30 Payment Terms need to fill out the
"Open a New Account Form " and fax it to 732-321-1542, or email it to sales@bricomedicalsupplies.net.

Established Customers wishing to fax Purchase Orders, can fax them to 732-321-1542 or email them to orders@bricomedicalsupplies.net.

**We Use D&B Resources and Contact References to determine credit worthiness prior to issuing credit to our customers.

***We charge a $35 Fee on all returned checks and charge a 1 1/2% Service Charge per month on any outstanding balance.
